Work order forms are the most downloaded document in maintenance and most of them are the wrong form. Search for one and you will get a contractor's billing document: customer name, address, rate, parts total, signature to authorise payment. That form exists to get somebody paid. Yours exists to record what was done to a machine you own, and the two share about half their fields. Delete the billing half
Customer, address, labour rate, parts pricing, total, authorisation signature. On an internal maintenance form these are dead weight, and they crowd out the fields that matter. If the form has no unambiguous asset field, it cannot be filed usefully and it is the wrong form.
Add the asset and the found field
The asset identifier is what everything hangs off. The found field is what turns a completed form into evidence you can use to change an interval. Two lines is enough; zero lines is the default on almost every template you can download.
Record hours honestly, not by half-day
The schedule is priced in hours: 66 technician hours a month on the worked example this site publishes. A form that collects 'morning' cannot feed that arithmetic, and the arithmetic is what defends the schedule in a budget round.
Blank forms are fine; filing them is the problem
A pad of printable work order forms works perfectly until somebody needs two years of history for one pump. That is when paper costs more than any subscription, and the honest test is whether anybody has ever successfully answered such a question from your file.
Questions people ask about work order forms
Is there a standard work order form?
No. Any template claiming to be standard is somebody's house style. What is consistent in good ones is the field list: asset, date, person, job, found, hours.
Should the form have a priority box?
Yes, with three or four defined levels rather than free text. Urgency written in words drifts between people within a month.
Can we mix paper and digital?
Briefly, during a transition, with a hard date. Two records of record kept in parallel is worse than either alone, because neither is complete.
